Description
Thiamine injection is a prescription form of Vitamin B1 primarily used to treat or prevent acute thiamine deficiencies, most notably Wernicke’s encephalopathy.
Uses and Indications
Wernicke’s Encephalopathy: High-dose parenteral administration is recommended by medical guidelines (such as the Royal College of Physicians) given intravenously or intramuscularly for a multi-day course to treat acute neurological symptoms linked to severe B1 deficiency.
Severe Deficiency: Thiofem 500 Injection is used when rapid replenishment is needed or when oral supplementation cannot be absorbed due to severe vomiting, malabsorption, or chronic alcohol misuse.
Mega- dose Thiamine is required beyond addressing “deficiency
High-dose Thiamine therapy is often necessary to overcome “metabolic block” & restore the deranged metabolism back to normal.

High dose Thiamine in TBI
A more recent study also looked at traumatic brain injury (TBI) with a focus on glutamate neuro excitotoxicity. They showed that excess nitric oxide and peroxynitrite found in neuroinflammation led to the inactivation of KGDH. KGDH inhibition reduced glutamate uptake into the Kreb’s cycle, producing glutamate excitotoxicity and neuronal cell death. Once again, extra levels of thiamine reversed this issue by stimulating KGDH, increasing glutamate clearance and protecting the cells against injury.
High dose Thiamine in Spinal cord injury
Spinal cord injury leads to significant neuroinflammation similar to that found in TBI, with excess nitric oxide production and deficits in brain glutathione levels (an intracellular antioxidant). Thiamine in high doses ameliorated excess nitric oxide levels and maintained brain levels of glutathione.

Thiamine injections completely reversed gait abnormalities and motor failure in two patients with Freidrick’s Ataxia.
High-dose thiamine in post-cardiac arrest
high-dose thiamine post-cardiac arrest restored pyruvate dehydrogenase activity in the brain, mitochondrial respiration, improved neurological function, reduced brain injury, and improved survival at 10 days. Pre-treatment with thiamine protected against cardiac ischemia by maintaining mitochondrial function, ATP concentrations, and inhibiting mitochondrial fission.
Thiofem 500 Inj is also used in Malabsorption induced by alcoholism, cirrhosis, GI disease in addition to Septic shock, Diabetic Ketoacidosis, Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS).

Administration and Safety
Route :
Given via slow intravenous (IV) infusion or deep intramuscular (IM) injection under direct medical supervision. For IV administration, large amounts are typically diluted in saline and given over a sustained duration to minimize risks.
Side Effects:
Generally safe, but rapid high-dose IV administration can rarely cause allergic reactions, drop in blood pressure, or localized tenderness at injection sites. It should never be self-administered.
