Aldofem Inj IV
High Potency Concentrate for Alcohol Detox.
It is highly likely that oral supplementation with thiamine will be inadequate in alcohol- dependent individuals who continue to drink. Therefore, parenteral thiamine is preferred for supplementation in deficiency states associated with chronic alcohol use.
Therapy involving parenteral thiamine is considered safe except for occasional circumstances of allergic reactions involving pruritus and local irritation.
Thiamine, also known as vitamin B1, plays critical roles in several biological processes, including the metabolism of glucose, synthesis of nucleic acids and reduction of oxidative stress. Thiamine deficiency could affect up to 70% of critically ill patients, and thiamine supplementation appears to increase lactate clearance and decrease the vasopressor dose.
Aldofem IV provides high concentration of Thiamine, Riboflavin, Nicotinamide,
Pyridoxine, Vitamin C along with anhydrous Dextrose & hence is a must in patients with septic shock or Alcohol detox.
Why Aldofem IV when Thiamine injections are available?
Most patients with deficiency of thiamine will also have reduced levels of other B vitamins including niacin, pyridoxine, and cobalamin that require replenishment.
For patients admitted to the intensive care unit with symptoms that may mimic or mask Wernicke encephalopathy, based on the published literature, routine supplementation during the 1st day of admission includes 200–500 mg intravenous thiamine every 8 h,( 2 pairs of Aldofem IV three times a day) & 64 mg/kg magnesium sulfate (≈4–5 g for most adult patients), and 400–1000 μg intravenous folate is required. If alcoholic ketoacidosis is suspected,dextrose-containing fluids are recommended over normal saline. Further, High-dose parenteral thiamine administered thrice daily has been advocated in patients at risk for Wernicke encephalopathy.
Why Aldofem IV for Liver diseases?
Hepatic disease ( Liver disease ) is often accompanied by hypovitaminosis due to an
increased need for vitamins in the face of decreased intake, intestinal malabsorption, and reduced hepatic storage capacity. Liver dysfunction can also prevent conversion of vitamins into their metabolically useful forms. Vitamin B complex depletion is common in hepatocellular disease.
Aldofem IV Preparation:
- Aldofem IV High Potency Injection – 2 x 5ml ampoules
- Ampoule 1 contains 250mg thiamine, 4mg riboflavin, 50mg pyridoxine.
- Ampoule 2 contains 500mg ascorbic acid, 160mg nicotinamide and 1000mg dextrose.
- The ampoule contents are separated due to long-term stability problems.
- Ampoule 1 and 2 must be mixed in a syringe prior to use.
- One pair refers to one of ampoule 1 and one of ampoule 2.
How Aldofem IV IS GIVEN:
Aldofem IV is be given by a healthcare professional by drip infusion into a vein. The product
comes in two ampoules, the contents of which are first diluted with either saline or 5%
glucose solution and then given over a period of 30 minutes.This medicine is for injection
into a vein only and should not be given by any other route. Dilute before use.
Dosage for adults including the elderly:
For rapid therapy of severe depletion or malabsorption of water soluble vitamins B and C, particularly in alcoholism: 2 to 3 pairs of 5 ml ampoules (1 pair = ampoule 1 + ampoule 2) diluted with 50 ml to 100 ml of infusion solution and injected over 30 minutes at intervals decided by your doctor (typically every 8 hours).For psychosis following unconsciousness from a narcotic drug (narcosis) or electroconvulsive therapy, or poisoning from infection: 10 ml of the mixed ampoules (1 pair) diluted with 50 ml to 100 ml of infusion solution and injected over 30 minutes. Injected twice daily for up to 7 days.
For haemodialysis patients:

Injected at the end of dialysis, once every 2 weeks.
Use in children and adolescents: Aldofem IV is not usually given to children; however, suitable doses according to the child’s age are:
- Age Dose under 6 years 1/4 of the adult dose
- 6-10 years 1/3 of the adult dose
- 10-14 years 1/2 to 2/3 of the adult dose
- 14 years and older one adult dose
The exact dose you will be given will be decided by the doctor who will monitor patient’s
condition and determine what treatment you need.
